The Clerk of the Board is responsible for keeping accurate minutes and maintaining Board records for each Board of County Commissioner (BCC) and Value Adjustment Board (VAB) meeting. In addition, every 4 years, the Clerk of the Board office provides staff support to the Charter Review Commission (CRC) appointed by the BCC. PROCUREMENT:

This amendment will add funding to expand outpatient ambulatory health services,  mental health services, oral health services, add a new Service Category:  Mobile Medical Clinic Services to the scope, and purchase a mobile medical unit to increase access to care for eligible Ending the HIV Epidemic clients including the aforementioned services.

 

FUNDING:

Funding is available in account number 7040-060-7302-8610. Funding provided via a Carryover Request of unobligated funds approved by the U.S. Department of Health and Human Services and Health Resources and Services Administration Ending the HIV Epidemic: A Plan for America-Ryan White HIV/AIDS Program Parts A and B grant.

APPROVALS:

The Health Services Department concurs with this recommendation.

REMARKS:

On September 22, 2020, the Board authorized award of Request for Proposals Y20-173A to CAN Community Health, Inc. in the estimated contract amount of $214,703 for a one-year term with four one-year renewals.

This project is supported by the Health Resources and Services Administration of the U.S. Department of Health and Human Services under the “Ending the HIV Epidemic”: A Plan for America - Ryan White HIV/AIDS Program Grant.  This contract is for qualified services performed to increase the number of eligible clients into care and viral suppression.  The additional funding will allow CAN Community Health, Inc. to expand outpatient ambulatory health services,  mental health services, oral health services, and assist with follow up visits.  The agency expects to serve an estimate of 200 additional clients with the additional funding.  The mobile medical unit will allow the agency to increase medical care to medically underserved HIV clients experiencing homelessness or who live in rural areas with little to no access to medical care and decrease barriers to care by expanding the service delivery models offered. The mobile medical unit will be staffed by medical professionals to provide direct medical, mental health, and oral health services throughout the Orlando Eligible Metropolitan Area. An estimate of 100 clients will be served through the mobile medical unit. CAN Community Health, Inc. will be required to purchase, use, manage, and dispose of the mobile medical unit in accordance with 45 CFR 75.320 Equipment.  The agency will report performance measures including clients served and viral suppression. 

Previous contract amendments are as follows:

Amendment No. 1: Renewed the contract for the period of October 1, 2021, through September 30, 2022.

Amendment No. 2: Increased the contract by $50,000 for a revised estimated contract amount of $264,703.

Amendment No. 3: Revised the term of the contract to October 1, 2021, through February 28, 2023, to align with the grant period.

Amendment No. 4: Deleted Amendment No. 3 in its entirety and corrected the renewal period of the contract to October 1, 2022, through February 28, 2023.

Amendment No. 5: Reduced the estimated contract amount from $214,073 to $110,292.92 for the reduced renewal period of October 1, 2022, through February 28, 2023.

Amendment No. 6: Renewed the contract for the period of March 1, 2023, through February 28, 2024.

Amendment No. 7: Revised the contract renewal period to March 1, 2023, through February 29, 2024.

Amendment No. 8: Increased the contract by $200,00 for a revised estimated contract amount of $464,703.

Amendment No. 9: Renewed the contract for the period of March 1, 2024, through February 28, 2025, and increased the contract by $300,000 for a revised estimated contract amount of $764,703.
