PROCUREMENT:

This amendment adds funds to continue services for the remainder of the current contract which ends on July 31, 2025.   Due to carryover funds received under this grant, a contract amendment is needed to increase funding to be able to continue to provide housing assistance and other services to youth and young adults in the Orlando Eligible Metropolitan Area. 

APPROVALS:

The Health Services Department concurs with this request.

 

REMARKS:

This program will continue to provide rapid rehousing services for the 41 youth clients with HIV currently enrolled in the program. These clients, experiencing homelessness or unstable housing, receive support through hotel/motel stays, rental and utility assistance, and other supportive services.  This additional funding will allow the program to expand services currently being offered to the clients enrolled, including additional rental and utility assistance to help clients achieve and maintain stable housing.

On January 9, 2024, the Board authorized award of Federal Subrecipient Agreement Y24-2201 with Zebra Youth, Inc. in the total estimated contract award amount of $238,590, for a one-year term and two additional one-year renewals.

Previous contract amendments are as follows:

Amendment No. 1:                       Increased the contract by $50,000, for a revised estimated contract amount of $288,590. Exhibit A - Scope of Services and Exhibit B - Budget were deleted and replaced in their entirety.

Amendment No. 2: Renewed the contract for the period of August 1, 2024 through July 31, 2025.

Amendment No. 3:                      Increased the contract by $50,000, for a revised estimated contract amount of $338,590.  Exhibit B - Budget was deleted and replaced in its entirety.
