PROCUREMENT:

This amendment allocates additional funding to assist low-income families by increasing access to crucial services like diaper assistance and supplies, food security for infants and toddlers, parenting education programs, and community resources. The implementation of the mobile diaper bank services further improves accessibility for families residing in remote or underserved areas who may not have convenient access to traditional brick-and-mortar locations.

FUNDING:

Funding is available in account number 5896-019-8613-8610.  Funding is provided from the American Rescue Plan Act of 2021 Coronavirus Local Fiscal Recovery Fund (the “Recovery Fund”).

APPROVALS:

The Citizens’ Commission for Children Division concurs with this request.

REMARKS:

On July 26, 2022, the Board authorized the award of Contract Y22-2410 to Central Florida Diaper Bank, Inc., in the total contract award amount of $264,262.86. This subaward was issued from the American Rescue Plan Act of 2021 Coronavirus Local Fiscal Recovery Fund (the “Recovery Fund”).

Previous contract amendments are as follows:

Amendment No. 1:                     Extended the subaward performance period from June 1, 2023, through August 31, 2023.

Amendment No. 2:                     Extended the subaward performance period through September 30, 2026. Replaced Exhibit A in its entirety by Exhibit A (Amendment #2).
