PROCUREMENT:
This change order will reallocate the remaining unused contract funds for the Bates Road Bridge Replacement and Roadway Project and adds funding for TLP Engineering Consultants, Inc. and its subconsultant, AtkinsRealis Group, Inc., to continue providing post-design services for this project. These post-design services for AtkinsRealis Group Inc. will be to address Requests for Information (RFIs) as needed, review shop drawings, and prepare minor modifications to the final construction plans, if necessary. Additional funding is requested due to increased volume of shop drawing reviews and RFIs, which have exceeded the level accounted for and included in the scope of Change Order No. 9. This has resulted in the accelerated depletion of post-design service funds, creating a need for additional funding to cover these additional post-design tasks. AtkinsRealis Group, Inc., expects that additional post-design services will be required for further structural review. Additionally, an update to the construction plans is expected at the completion of construction for this project. The requested funding via Change Order No. 10 will allow the project team to complete the necessary post-design services and provide the County with accurate and comprehensive as-built documentation upon completion of the project.

Funding is available in account number 1003-072-2852-6311.
APPROVALS:
The Public Works Engineering Division concurs with this recommendation.
REMARKS:
On October 9, 2014, Task Authorization No. 8 was created for the final engineering design services for the construction of the Bates Road Replacement Bridge Project in the amount of $241,490.83.
Previous change orders for this task authorization are as follows:
Change Order No. 1: Revision to the original scope of service, additional structural and bridge analysis, preparation of Letter of Map Revision, and additional environmental and permitting services in the amount of $93,829.21, for a revised total amount of $335,320.04.
Change Order No. 2: Addition of 498 days to final completion date to complete the remaining items included in this Task Authorization. The total of purchase order remained the same.
Change Order No. 3: Revision to the original scope of service based on the requested additional utility coordination, survey update, environmental and geotechnical services, in the amount of $47,531.48, for a revised total amount of $382,851.52.
Change Order No. 4: Addition of 1,049 days to final completion date to complete the remaining items included in this Task Authorization.
Change Order No. 5: Addition of 399 days to final completion date to complete the remaining items included in this Task Authorization.
Change Order No. 6: Redesign and update the construction plans to reference the current FDOT Standards Plans. Also, it includes the additional utility coordination to prepare lighting plans which were not included under the original contract, and resubmittal of the St. John’s River Water Management District (SJRWMD) permit. Change order also added 365 days to the final completion date, in the amount of $65,066.58, for a revised total amount of $447,918.10.
Change Order No. 7: Prepare and submit a State 404 Dredge and Fill Permit from the Florida Department of Environmental Protection. Also, update project specific information in the narrative provided by SJRWMD and publish the narrative in the Orlando Sentinel. Change order added 120 days to the final completion date, in the amount of $16,515.91, for a revised total amount of $464,434.01.
Change Order No. 8: To include Permitting Fees, including the Army Corp of Engineers permit and to mail the Construction Newsletter (Newsletter #3), in the amount of $2,774.99, for a revised amount of $467,209.
Change Order 9: Reallocate remaining unused contract funds to Post-Design Services for TLP Engineering Inc. and its subconsultants, to address RFIs as needed. This change order did not increase contract amount.
