PROCUREMENT:
This amendment adds funding for the remainder of the contract term, to expand services for the service categories, Lot 1: Tenant-Based Housing Assistance, Lot 2: Short-Term Rent, Utility, and Mortgage Assistance; Lot 3: Permanent Housing Placement; and Lot 5: Facility Based Housing Assistance for eligible HOPWA clients.
FUNDING:
Funding is available in account number 7738-060-7312-8610. Funding for this program comes from the City of Orlando HOPWA grant from the U.S. Department of Housing and Urban Development.
APPROVALS:
The Health Services Department concurs with this recommendation.
REMARKS:
On September 12, 2023, the Board authorized the award of Contract Y23-2506C to the Center for Multicultural Wellness and Prevention Inc., in the total contract award amount of $1,343,151 for a one-year term with two one-year renewals. It is estimated that additional funding will allow the Center for Multicultural Wellness and Prevention Inc. to expand services to up to 25 households for the performance period of October 1, 2024 through September 30, 2025.
Previous contract amendments are as follows:
Amendment No.1: Added language to the agreement for cost reimbursement payment; replaced Exhibit A-4 Scope of Services in its entirety with Exhibit A-4 (Rv.1).
Amendment No. 2: Corrected an erroneous revision within Amendment No.1; revising the Unit Rate of Exhibit A-4 (Rv.1) from $34.38 to $45.40 for a 2-bedroom unit and $51.51 for a 3-bedroom unit.
Amendment No. 3: Increased the contract by $16,849, for a revised total contract amount of $1,360,00, to expand housing assistance to eligible HOPWA clients; replaced Exhibit B - Expenditure of Funds with Exhibit B (Rv.1).
Amendment No. 4: Renewed the contract for the period of October 1, 2024, through September 30, 2025; added the Human Trafficking provisions to the contract; increased the Orientation Flat Rate from $7,100 to $7,313 and the Fixed Price Rate from $12.98 to $13.37 per unit for "Exhibit A-1" Lot 1-Tenant-Based Rental Assistance, "Exhibit A-2" Lot 2-Short-Term Rent, Utility and Mortgage Assistance, "Exhibit A-3" Lot 3-Permanent Housing Placement, "Exhibit A-5" Lot 5-Facility Based Housing Assistance, "Exhibit A-6" Lot 6-Supported Services/Case Management. All Exhibits as titled are replaced in their entirety inclusive of their original title as (Rv.1) [Ex. "Exhibit A-1 (Rv.1)"]
Amendment No.5: Added language to Lot 5 Scope of Services, to include Section IV. Payment, Item 3. (a) unit rate from $34.38 to $45.50 for a 2-bedroom unit and $51.51 for a 3-bedroom unit; replaced Exhibit A-5 (Rv.2) with Exhibit A-5 (Rv.3).
