PROCUREMENT:
This amendment will provide additional funds needed to provide occupational medical services for Orange County employees due to increased hiring activities.
FUNDING:
Funding is available in account number 5510-025-0328-3195.
APPROVALS:
The Risk Management Division concurs with this recommendation.
REMARKS:
The Board authorized award of Y23-134 on July 11, 2023 to Adventist Health System/Sunbelt, Inc. dba AdventHealth Centra Care in the estimated contract amount of $3,033,210 for a three-year term, with further approval for the Procurement Division to renew the contract for two additional one-year terms.
Previous contract amendments are as follows:
Amendment No. 1: Replaced Exhibits B.2, B.3, B.5, B.8, and B.19 with Exhibits B.2 Rev, 1, B.3 Rev. 1, B.5 Rev. 1, B.8 Rev. 1, and B.19 Rev. 1 respectively.
Amendment No. 2: Added a provision allowing use of contract by other government agencies to allow piggybacking.
Amendment No. 3: Replaced Exhibit B.18 with Exhibit B.18 Rev. 1.
Amendment No. 4: Replaced Exhibit B.19 Rev. 1 with Exhibit B.19 Rev 2.
Amendment No. 5: Replaced Exhibit B.19 Rev. 2 with Exhibit B.19 Rev 3.
Amendment No. 6: Replaced Exhibit B.18 Rev. 1 with Exhibit B.18 Rev. 2.
Amendment No. 7: Replaced Exhibit B.19 Rev. 3 with Exhibit B.19 Rev 4 and increased the contract by $303,321 for a revised estimated contract amount of $3,336,531.
