Approval of Amendment No. 3, Contract Y25-2006, Provision of Community Services, with Community Health Centers, Inc., in the amount of $251,908.84, for a revised total contract amount of $1,264,558.56. ([Community and Family Services Department Citizens' Commission for Children Division] Procurement Division)

PROCUREMENT:
This amendment will provide additional funds for pediatric expansion and youth dental services, resulting in an additional 1,968 medical/dental care visits and more than 1,000 additional residents served.
Programming provides uninsured, or underinsured, Orange County youth ages 0 through 18, with pediatric medical and dental care. Households with combined incomes at or below 200% of the Federal Poverty Level are eligible for services on a sliding discount program.
Community Health Centers, Inc. (CHC) is currently providing satisfactory services, and programmatic monitoring has demonstrated contract compliance. The additional funding will be sourced from Children's Services Rollover Funding and must be utilized by the conclusion of the contract year ending September 30, 2026.

Funding is available in account number 0001-062-2556-8610.
APPROVALS:
The Citizens' Commission for Children Advisory Board recommended the investment of Children's Services funding in community-based medical and dental services for youth with CHC. The Citizens' Commission for Children Division (CCC) concurs with the funding recommendation.
