Title: Approval and execution of (1) Embrace Families Community Based Care Standard Contract No. OROS016-1920 between Embrace Families Community Based Care Inc. and Orange County, Florida for Family Preservation and Stabilization Program in the amount of $458,141.04 for the period of July 1, 2019 through June 30, 2020; (2) Certification Regarding Lobbying Certification for Contracts, Grants, Loans and Cooperative Agreements; (3) Debarment, Suspension, Ineligibility and Voluntary Exclusion Contracts/Subcontracts; (4) Affidavit; (5) State and Federal Single Audit Act Certification of Exemption; (6) HIPAA Business Associate Agreement; and (7) Certification of Adequate Insurance Coverage; and authorization for the County Mayor or designee to approve any increases, decrease, or amendments to this contract.
