Approval of Amendment No. 4, Contract Y21-2004, Provision of Community Services and Facility Use, with Capoeira for Tomorrow, Inc., in the amount of $150,000, for a revised total contract total of $417,500. PROCUREMENT:
This amendment will provide additional funding for the extension period executed via Amendment No. 3 for the period of July 1, 2024 through December 31, 2024. The additional funding ensures continuity of services accompanied by calendar year funding. Programming provides comprehensive fitness programming, to include wellness and nutrition education components, with a focus on the overall health and holistic wellbeing of program participants. The funds authorized through this amendment will be utilized to provide an additional 13,043 additional hours of services to the community through the end of the extended contract term.
FUNDING:
Funding is available in account number 0001-062-2556-8610.
APPROVALS:
In accordance with the Additional Funding Guidelines approved by the Citizens' Commission for Children Advisory Board, contracted agencies may be considered for additional funding on a case-by-case basis, in the event that the agency has expended 75% or more of their funding by mid-year and has been monitored to ensure contract compliance. The Citizens' Commission for Children Division concurs with this recommendation
